I reckon Ivan is a scientist who is making people who have been severely injured into cyborgs. Alfred was hit by a car while out walking one day and was so severely injured he couldn't move, so the doctors sent him to Ivan's laboratory via ambulance. When Alfred woke up he found he was now a cyborg. This amazing picture shows him getting repairs done to his back.
